What the performance evaluation form is

A formal performance evaluation is a structured way to assess how an employee has performed over a defined period. This form gives you a single record for the whole review: it captures the employee's details and the evaluation period, scores performance against a consistent set of criteria, and produces two summary outcomes — a total percentage score and an overall performance rating drawn from the scale Outstanding, Exceeds expectations, Meets expectations, Needs improvement, or Unsatisfactory. The form is organised into three sections. In Performance Evaluation you record the basic facts of the review. In Performance Evaluation Criteria you score the substance of the employee's work. In Completion you record the overall rating, comments, recommendations, and signatures.

Why a consistent evaluation process matters

Assessing performance on a structured scale rather than from memory reduces inconsistency between reviewers. By scoring every criterion on the same 5-to-1 scale, you can compare results fairly, sum them to a total percentage, and identify areas for praise and areas requiring correction. A completed evaluation also feeds forward into development and corrective action, so the review has a practical outcome rather than serving as a record alone.

What the evaluation covers

The form scores the employee on nine criteria: - **Attendance and punctuality** — consistency in arriving on time and meeting scheduled commitments - **Job knowledge and skills** — command of the technical and practical requirements of the role - **Quality of work** — accuracy, thoroughness, and consistency of output - **Initiative and motivation** — drive to act independently and pursue improvement - **Productivity** — volume and efficiency of work completed against expectations - **Teamwork** — collaboration with colleagues and contribution to shared goals - **Team goal attainment** — contribution to meeting the team's objectives for the period - **Overall conduct** — professional behaviour and adherence to workplace expectations - **Discipline** — compliance with company rules, procedures, and directions Each criterion is rated on a scale of 5 (highest) to 1 (lowest). Add the scores for every criterion to get the total percentage achieved, then use that total to choose the overall performance rating.

How to carry out the evaluation

Work through the form in order: 1. Capture the administrative details — conducted on, prepared by, location, employee ID, hire date, employment status, evaluation period start and end dates, and evaluator name and role. 2. Score each of the nine criteria on the 5-to-1 scale, using evidence from the evaluation period. 3. Sum the scores and record the total percentage achieved. 4. Review the total and select the matching overall performance rating. 5. Record any special task undertaken or commendation earned, and write general comments covering areas of praise and areas requiring correction. 6. Note any recommended development or corrective actions. 7. Collect signatures to close out the review, then note the completion date metadata.

How to get the most from the form

Score each criterion on the evidence you gathered during the period, not on how the review feels in the moment. Write comments that are specific enough to act on, distinguishing clearly between praise and required correction. Leave the overall rating blank until you have summed every criterion, so the rating follows the score rather than the other way around. The form is 100% customizable, so you can adjust it to match your company's standard rating scale. If you do, keep the scoring, the total, and the overall rating consistent with each other.

Tips

- Define the evaluation period clearly before you begin, and stick to that window when gathering evidence. - Score all nine criteria on the same scale so that the total percentage is comparable across employees. - Use the general comments field to record both areas of praise and areas requiring correction. - Set out recommended development or corrective actions so the employee has a clear next step. - Keep a signed copy of the completed evaluation in the employee's record.

Frequently Asked Questions

You can use this performance evaluation form in a periodic review cycle to assess an employee's overall performance, identify areas of praise, and flag areas that need correction. It covers attendance and punctuality, job knowledge and skills, quality of work, initiative and motivation, productivity, teamwork, team goal attainment, overall conduct, and discipline.

Each criterion is rated on a score of 5 (highest) down to 1 (lowest). To complete the form, you sum the scores awarded for every criterion and record the total percentage achieved, then review that total and select the overall performance rating that corresponds to it.

Insert the employee's details — employee ID, hire date, employment status, and the evaluation period start and end dates — and record who conducted the evaluation and when. The employee's name and role come from the signature area once the review is complete.

The form includes sections for special tasks undertaken or commendations earned, general comments on performance (areas of praise and areas requiring correction), and recommended development or corrective actions.

The form provides two rating scales. The first is a numerical scale (5 to 1) for scoring each individual criterion. The second is a descriptive scale — Outstanding, Exceeds expectations, Meets expectations, Needs improvement, Unsatisfactory — used to select the overall performance rating once the total percentage score has been reviewed.

Yes. The template is 100% customizable, so you can modify it according to your company's standard rating scale. If you change the rating scale, make sure the scores awarded for each criterion still sum to the total percentage you intend to record, so the total and the overall rating remain consistent.
