What OEE reporting is

Overall Equipment Effectiveness (OEE) is a measure of how effectively manufacturing equipment is used. To improve it, you need to know where production time and output are lost. The six major losses fall into three groups: downtime, speed, and quality. Downtime losses are split into unplanned downtime and planned downtime. Speed losses are split into minor stops and slow cycles. Quality losses are split into production rejects and initial rejects. This report template gives machine operators and quality managers a structured way to identify and document these losses in daily manufacturing operations.

Why it matters

Recording the six major losses gives you a consistent picture of where equipment and time are being lost. Without that record, losses tend to be handled case by case and the underlying causes are hard to see. Documenting each loss as it occurs supports more informed decisions about maintenance, changeovers, materials planning, and quality.

What a good OEE report covers

A good report identifies each of the six major losses and captures the detail behind it. The template organises the work into the following sections, with the items listed under each.

Unplanned Downtime

Capture equipment failures and breakdowns, unplanned repairs, material shortages, and a lack of transportation for completed goods. Also record whether equipment installation took longer than expected.

Planned Downtime

Record changeovers that took longer than anticipated and operator absence, such as breaks or excessive time away from the machine.

Minor Stops

Note any minor slowdowns in operations, incorrect operation performed on the equipment, and a lack of maintenance.

Slow Cycles

Identify wear evident on the equipment and incorrect settings applied to it, such as adjusting speed slower than optimal.

Production Rejects

Record products that could not be salvaged and products that needed rework to be made usable.

Initial Rejects

Note initial losses during the production ramp-up phase.

How to carry out the report

The template is mobile-friendly so it can be completed on the floor as work happens. Start by recording who conducted the report and who prepared it, the location, the inspection real start date, and the inspection real end date. You can then work through the loss categories in order — unplanned downtime, planned downtime, minor stops, slow cycles, production rejects, and initial rejects — answering each item as it applies to the operation. The items use a simple Yes/No answer scale. Finish by adding a signature and the finished-at time. With 22 items in total, the report can be completed during or immediately after the production run while the details are still fresh.
