How to Use This Checklist
- **Record the basics.** Note the date conducted, who prepared the assessment, and the location, so the evaluation is tied to a specific time and place.
- **Set the frame.** Before scoring, clarify the role the candidate is being assessed against and what the job demands in terms of productivity, quality of work, attitude, and punctuality.
- **Score every criterion.** Work through knowledge of the role, productivity, quality of work, technical skills, communication, creativity, enthusiasm, cooperation, attitude, initiative, punctuality, attendance, consistency, professional relationships, and reliability, rating each as Excellent, Good, or Poor.
- **Assess general skills as a whole.** Bring the individual criteria together into an overall view of the candidate's general skills for the role.
- **Close with recommendations.** Summarize your findings and give an overall skills rating, supported by the scores you recorded.
- **Sign and date.** Add your signature, the time finished, and the actual inspection start and end dates so the assessment is traceable.

Why it matters
An assessment built only on impressions is hard to defend and easy to get wrong. Scoring defined criteria on a common scale gives you a record you can return to, compare across candidates, and justify to the people involved. It also makes the same standard apply to everyone being reviewed for a role. The template reflects this by asking for a rating on each criterion and by asking you to close with recommendations and an overall rating, rather than stopping at the scores themselves.
What a good assessment covers
A useful skills assessment looks at more than whether someone can do the technical part of the job. The criteria in this template fall into areas that together describe how a person performs in a role: - **Knowledge of the role** — whether the candidate understands the work itself. - **Productivity and quality of work** — how much gets done and how well. - **Technical skills and communication** — the craft of the job and the ability to work with others through it. - **Creativity and initiative** — the willingness to find and act on better approaches. - **Enthusiasm, cooperation, and attitude** — how the person engages with the work and the team. - **Punctuality and attendance** — reliability in showing up and starting on time. - **Consistency, professional relationships, and reliability** — how steady and trustworthy the person is over time. Each of these is rated as Excellent, Good, or Poor, so the individual criteria add up to a picture rather than a single impression.
How to carry out the assessment
Record the date conducted, who prepared the assessment, and the location first. Then go criterion by criterion, scoring honestly against the demands of the role rather than against an abstract ideal. Where a criterion is unclear, return to what the job actually requires — productivity, quality of work, attitude, and punctuality are a reasonable starting frame. Finish by giving recommendations and an overall skills rating. A score without context is harder to use, so explain briefly what sits behind it. Sign and date the form, note the time finished, and record the actual inspection start and end dates so the assessment can be traced back to the moment it happened. The template is fully customizable, so you can adjust the criteria or the scale to fit the role you are assessing.
