How to Use This Checklist

  • Work through every question with the group before starting, not after. The point of the assessment is to confirm understanding before work commences.
  • Answer each question openly. If something is not yet in place, record it as No so it can be resolved rather than assumed.
  • Use N/A only where a question genuinely does not apply to the task at hand, and be prepared to explain why.
  • Give every group member the chance to speak. The checklist asks whether everyone has been consulted on task requirements and the safest methods, and that includes hearing concerns.
  • Resolve any No answers before the task starts. If a hazard, control, or piece of equipment is not ready, address it first.
  • Record the signature, start and finish times, and the actual inspection start and end dates so the assessment stands as a complete record.

What a Take 5 group safety assessment is

A Take 5 is a short, structured safety assessment carried out before work begins. In a group setting, the whole team walks through the same set of questions together and confirms, out loud and on the record, that they understand the hazards, emergency arrangements, and controls for the task ahead. This checklist is built for that group process. Rather than one person completing it alone, it is designed to be worked through with everyone present before any tasks commence. That matters because the questions themselves are about shared understanding: whether all group members have completed site induction, whether everyone knows how to report hazards, incidents, and near-misses, and whether everyone has been consulted on how the work should be done safely. When the assessment is done with the group, gaps in knowledge surface while there is still time to close them.

Why it matters

Workplace tasks rarely go wrong because of a single missing control. More often, something is assumed: that someone else checked the equipment, that everyone knows the evacuation route, or that the hazards were discussed at some earlier point. A group Take 5 removes those assumptions by asking directly and recording the answer. The checklist covers the areas where assumptions are most costly. It asks whether safety policies and procedures are understood, whether emergency facilities and escape routes are established and communicated for this location, whether first aid equipment is accessible and in good working condition, and whether hazardous substances are used and stored in accordance with their Safety Data Sheets (SDS). Each of these is a question that should have a definite answer before work starts, not a probable one.

What a good assessment covers

A complete pre-work assessment checks the people, the place, the equipment, and the plan. **People and understanding** - Required site induction training completed by all group members - Awareness of the organization's health and safety policies and procedures - Understanding of how to report hazards, incidents, and near-misses - Consultation with workers about task requirements and the safest methods - Appropriate personal protective equipment (PPE) for the specific task **The site and emergency arrangements** - Emergency facilities, evacuation procedures, and escape routes established and communicated for this location - Emergency and first aid equipment accessible and confirmed to be in good working condition - Consultation with the site supervisor on site-specific hazards and risk controls - Safe and unobstructed access and egress to and from the work area - Adequate separation between moving vehicles and personnel **Equipment and substances** - Correct safe work procedures and appropriate equipment available and ready - Electrical and mechanical equipment inspected and confirmed safe - Hazardous and dangerous substances used and stored in strict accordance with their Safety Data Sheets (SDS) **Physical demands of the task** - Manual handling risks identified, assessed, and controlled

How to carry out the Take 5

1. **Gather the whole group before any task starts.** The assessment is a group activity by design. Everyone who will do the work should be present. 2. **Record the basics.** Note the date conducted, who prepared the assessment, and the location. 3. **Work through the questions in order, together.** Answer each one Yes, No, or N/A. Ask the group rather than answering on their behalf, particularly for the questions about induction, policy awareness, and hazard reporting. 4. **Confirm emergency arrangements explicitly.** Cover the emergency facilities, evacuation procedures, and designated escape routes for this location, and check that the first aid and emergency equipment is accessible and in working condition. 5. **Check the site-specific hazards.** Make sure the group has consulted the site supervisor about the hazards and risk controls relevant to this work. 6. **Verify the equipment and substances.** Confirm safe work procedures and equipment are ready, that electrical and mechanical equipment has been inspected, and that any hazardous substances are being handled in line with their Safety Data Sheets. 7. **Cover access, vehicles, and manual handling.** Confirm safe access and egress, separation between vehicles and personnel, and that manual handling risks have been identified and controlled. 8. **Deal with every No before starting.** A No is not a formality. It identifies something to resolve first. 9. **Close out the record.** Capture the signature, the finished time, and the actual inspection start and end dates.

Frequently Asked Questions

A Take 5 is a short, documented safety assessment carried out before work begins. It involves walking through a set of safety questions with the whole team so that everyone confirms, together, that they understand the site hazards, emergency arrangements, and the controls in place for the task ahead.

It is designed for team settings. The idea is that the assessment is completed as a group before any tasks start, rather than by one individual on everyone else's behalf, so that each member can ask questions, raise concerns, and confirm their understanding.

The checklist records the date conducted, who prepared it, and the location, along with signatures, start and finish times, and the actual start and end dates of the inspection.

The checklist asks about site induction training, awareness of health and safety policies, hazard and incident reporting, emergency facilities and evacuation routes, first aid equipment, site-specific hazards, safe work procedures and equipment, separation of vehicles and personnel, inspection of electrical and mechanical equipment, storage and use of hazardous substances against their Safety Data Sheets, worker consultation, personal protective equipment, access and egress, and manual handling risks.

The checklist uses a Yes, No, or N/A response for each question. This gives a clear record of what was confirmed, what was not, and what did not apply to the task.
