How to Use This Checklist
- Work through the STOP questions first: confirm you are trained, competent, and authorised for the task, fit for duty, that you understand the requirements, that a current procedure or Safe Work Practice is available, and that you know the emergency response procedures.
- Carry out the LOOK stage and note what you find. Where a hazard is present or the situation is at risk, complete a full hazard assessment or an isolation assessment as directed.
- Record the total number of hazards identified in the work area and rate the overall residual risk after controls are applied on a scale of 1 to 10.
- Document any additional controls implemented, observations, or follow-up actions required, and complete the signature and time fields.

Second person: you
SLAM is a method for stopping, looking, assessing, and managing risk before and during a task so hazards are identified, evaluated, and controlled, keeping you and those around you safe.
Working through a task
**STOP** — before you reach for a tool, confirm you are trained, competent, and authorised for the task, physically and mentally fit for duty, clear on the task requirements and that the work has been properly planned. Check that a current procedure or Safe Work Practice is available, that you hold the right tools and PPE, and that you know the emergency response procedures. If any answer is no or not applicable, stop and consult your supervisor before continuing. **LOOK** — examine the work area for hazards. The checklist prompts you to consider hazardous forms of energy such as electrical, mechanical, thermal, or chemical energy, whether energy isolation or lockout/tagout is required, slip, trip, and fall hazards, housekeeping, the risk of being caught in, on, or between objects or machinery, communication with the team, working near mobile plant or moving equipment, whether barricading or access control is needed to protect others, and whether you could be injured or introduce hazards to others during the task. Where a hazard is present, complete a full hazard assessment, and where isolation is required, complete an isolation assessment. **ASSESS** — evaluate how identified hazards could affect people, property, and the environment. Count the total number of hazards in the work area, decide whether it is safe to proceed, and rate the overall residual risk after controls are applied on a scale of 1 (low) to 10 (critical). **MANAGE** — put effective controls in place, communicate them to everyone involved, and document any additional controls implemented, observations, or follow-up actions required.
