How to Use This Checklist

  • **Set up before you start.** Confirm your location, date, and who is prepared by, and check that every item on the equipment and supplies list is present — bleach disinfecting wipes, a doffing pad, duct tape, examination gloves, protective goggles, alcohol-based hand sanitizer, long-cuff nitrile gloves, an N95 respirator or PAPR, a disposable plastic apron, surgical boot covers or dedicated boots, a designated trash receptacle, and a Tyvek coverall suit.
  • **Follow the donning procedure in order.** The checklist walks you through each layer: donning the coverall without zipping, adding boots or shoe covers and taping them with a pull tab, sanitizing hands, layering inner and outer gloves, fitting and sealing the N95 respirator, placing goggles over it, extending the hood, and taping the zipper. Each step includes the technique that makes it work — bending at the knees or elbows before taping, pressing the nose clip to tighten the seal, and checking for gaps.
  • **Carry out the doffing procedure deliberately.** Remove the outer apron and outermost gloves, wipe down the suit with bleach disinfecting wipes before leaving the patient care area, and have a buddy inspect the ensemble for degradation, tears, or breaches. Remove the hood, goggles, and respirator, then use the pull tabs to release the tape and remove the suit from shoulders, arms, and legs. Step clearly away from the pile of contaminated materials.
  • **Record and sign off.** Note any procedural observations, capture the start and end times, and sign the form so the inspection is traceable and accountability is clear.

What this checklist covers

The PPE Checklist and Procedures is a 45-item tool for preparing, donning, and doffing personal protective equipment. It is designed for emergency response and infection control work, where engineering and administrative controls may not be enough on their own to keep exposure risk down. The checklist is organised into four sections. **PPE Inspection and Procedures** captures the basic facts of the inspection and who prepared it. **PPE Equipment and Supplies Checklist** confirms that every required item is present. **Donning PPE Procedure** takes you through each step of putting the equipment on. **Doffing PPE Procedure** covers removing it without contaminating yourself or your surroundings. Each supply and procedural item uses a simple **Yes / No / N/A** response so you can record what is actually true on the day.

Why it matters

PPE only protects you when it is complete, correctly fitted, and removed in the right order. A missing seal on a respirator, an untaped glove cuff, or an unplanned doffing sequence can let contamination reach skin or clothing. This checklist addresses those risks by making every step explicit. It prompts you to verify the seal of an N95 by pressing along the metal nose clip, to confirm that goggles do not open a gap in that seal, and to check that outer gloves fully cover the suit. During doffing, it calls for a buddy to inspect the ensemble for degradation, tears, or breaches before the equipment comes off, and it walks you through a sequence that keeps the contaminated exterior away from exposed skin.

What a good inspection includes

A thorough PPE check has four parts, which map to the four sections of this checklist: 1. **Inspection details.** Record the date, preparer, and location so the check is traceable. 2. **Supplies on hand.** Confirm the availability of each item — bleach disinfecting wipes, a doffing pad, duct tape, examination gloves, protective goggles, alcohol-based hand sanitizer, long-cuff nitrile gloves, an N95 respirator or PAPR, a disposable plastic apron, surgical boot covers or dedicated boots, a designated trash receptacle, and a Tyvek coverall suit. 3. **Donning.** Work through every step in order, from the coverall and boots through to the respirator, goggles, hood, and sealed zipper. 4. **Doffing.** Remove each layer in the correct sequence, wipe down the suit before exiting the patient care area, have a buddy inspect it, and step away from the contaminated materials.

Donning procedure in detail

The donning section guides you through the following sequence: - Put on the Tyvek coverall suit without zipping it, in a size that allows unrestricted movement. - Use a hoodless suit, or tuck the hood securely into the back of the suit. - Put on boots or shoe covers, then tape around them. Bend your knees while taping so you keep full range of motion, and create a pull tab connected to the upper boot for easy removal. - Apply alcohol-based hand sanitizer to clean both hands thoroughly. - Put on the inner layer of gloves and pull the suit cuffs over each glove. - Put on the longer outer layer of gloves, making sure it fully covers the Tyvek suit so no skin is exposed. - Tape the outer glove to the suit sleeve using the same method as the boots — bend at the elbow and leave a pull tab. - Position the N95 respirator over your face and tighten the seal by pressing down and inward with two fingers along the metal nose clip. An optimal seal may take up to five minutes to establish, so take your time. - Place protective goggles over the respirator and check that they do not create an open gap that compromises the seal. - Extend the hood over your head and inspect all edges for gaps that may need extra taping. - Seal the suit zipper with tape, leaving a pull tab at the top for doffing. - If necessary, tape the exterior of the hood to the goggles or respirator for an additional layer of security.

Doffing procedure in detail

The doffing section follows this order: - Remove the outer apron if one was worn. - Remove the outermost pair of gloves if applicable. - Wipe down the suit surface and any visible contaminants with bleach disinfecting wipes before exiting the patient care area. - Have a buddy inspect the full PPE ensemble for any signs of degradation, tears, or breaches. - Remove the hood carefully so the exterior of the suit does not touch exposed skin or hair. - Remove the goggles and the N95 respirator. - Use the pull tabs to carefully remove the tape from the zipper. - Unzip the suit and roll the shoulders out of the garment. - Withdraw your arms from the suit, using the tape pull tabs at the wrists to release your hands from the outer gloves if needed. - Remove your legs from the suit, touching only the inside of the suit material with the inner clean glove. - Step clearly away from the pile of contaminated materials.

Final sign-off

The checklist closes with a **Signature**, a **Finished At** time, and the **Inspection real start date** and **Inspection real end date**. These fields turn the record into an accountable document: they show that the procedure was completed, when it started, when it ended, and who verified it.

Frequently Asked Questions

PPE stands for personal protective equipment. It is the gear you wear — such as N95 respirators, nitrile gloves, goggles, and coveralls — when engineering and administrative controls alone are not enough to reduce exposure risk. The checklist is built around infection control and emergency response scenarios, where close contact and contamination are hard to avoid.

Yes. Before you begin donning, confirm that all required supplies are on hand. The checklist covers bleach disinfecting wipes, a doffing pad, duct tape, examination gloves, protective goggles, alcohol-based hand sanitizer, long-cuff nitrile gloves, an N95 respirator or PAPR, a disposable plastic apron, surgical boot covers or dedicated boots, a designated trash receptacle, and a Tyvek coverall suit. If any item is missing, resolve that before proceeding.

Work through it in order. Don the Tyvek coverall without zipping, using a hoodless suit or tucking the hood into the back. Put on boots or shoe covers, tape around the boots with a pull tab, and sanitize your hands. Add the inner gloves and pull the suit cuffs over them, then add the longer outer gloves so they fully cover the suit. Tape the outer gloves to the sleeves with pull tabs. Fit the N95 respirator and press along the metal nose clip to tighten the seal, then place goggles over it without creating a gap. Extend the hood and check all edges for gaps, seal the zipper with tape and a pull tab, and add extra tape at the hood if needed.

Remove your outermost gloves and any apron worn. Wipe down the suit and visible contaminants with bleach disinfecting wipes before leaving the patient care area. A buddy then inspects the full ensemble for degradation, tears, or breaches. Remove the hood carefully so the exterior does not touch exposed skin or hair, then remove the goggles and N95 respirator. Use the tape pull tabs to release the zipper, unzip the suit, roll the shoulders out, withdraw your arms, and remove your legs — using only the inner clean glove to touch the inside of the suit. Step clearly away from the pile of contaminated materials.

Yes. A buddy inspects the full PPE ensemble for signs of degradation, tears, or breaches during doffing. This step helps catch failures before contaminated gear is removed.
