What this checklist is for
A safety meeting only does its job if you can show what was covered and who was there. This sign-in sheet is the record of that: it captures the meeting details, the attendance, and the outcome in one place, so briefings can be repeated on a regular schedule and the completed records kept for compliance tracking and long-term documentation. The checklist is organised into three sections — **Safety Meeting Information**, **Attendee Sign-In**, and **Discussion Summary & Follow-Up** — across 20 items.
What a complete record covers
A complete record covers three things. - **Meeting information** — who prepared the record and where it was done (**Conducted on**, **Prepared by**, **Location**), and the meeting itself (**Meeting Topic or Subject**, **Meeting Date**, **Meeting Location**, **Facilitator or Presenter Name**). - **Attendance** — for every attendee: **Attendee Full Name**, **Department or Team**, **Job Title or Position**, **Attendance Status**, and **Signature**. Attendance is recorded against a fixed set of statuses: **Present**, **Absent**, **Late Arrival**, **Early Departure**, or **Excused**. Department is selected from **Operations**, **Maintenance**, **Safety & Compliance**, **Administration**, **Engineering**, **Warehouse**, **Quality Control**, or **Other**. - **Outcome and follow-up** — **Topics Discussed**, **Key Decisions and Action Items**, the **Total Number of Attendees**, a **Meeting Effectiveness Rating**, and the **Next Scheduled Meeting Date**. The record is then closed off with **Finished At** and the real start and end dates of the session.
How to run it
Complete the **Safety Meeting Information** block before the meeting begins, so the topic, date, location, and facilitator are already captured. During the meeting, work through **Attendee Sign-In** — either as people arrive or as the sheet is passed round — and make sure each person's name, department or team, job title, and signature are recorded, and that their **Attendance Status** is set. Once the discussion is finished, complete **Discussion Summary & Follow-Up**: summarise **Topics Discussed**, note the **Key Decisions and Action Items**, count the **Total Number of Attendees**, give a **Meeting Effectiveness Rating**, and set the **Next Scheduled Meeting Date**. Finish by noting **Finished At** and the real start and end dates. Keep the completed records so recurring briefings can be tracked over time and the documentation stays consistent.
