How to Use This Checklist
- Identify the employee and the respirator. Record the employee identification details and the respirator's manufacturer and model number so the test is tied to both the wearer and the equipment they will actually use.
- Confirm the wearer is ready. Check that the wearer is free from facial hair that could interfere with the respirator face seal, and note whether the wearer donned the respirator independently without assistance.
- Complete the hazard assessment. Work through the hazard assessment for construction dust and fiber particles and record the date conducted, the person who prepared it, and the location.
- Apply the sensitivity solution and establish the sensitivity test threshold level before the exercises begin.
- Run the seven fit test exercises in order. For each, record a pass or fail: normal breathing, deep breathing, turning the head side to side, moving the head up and down, bending over, talking (reading aloud), and a final normal breathing check.
- Record the total test duration in minutes, set the overall fit quality rating, and note any additional observations.
- Sign the record and capture the time the test was finished, along with the inspection real start and end dates, so the completed record stands as evidence in accordance with respiratory protection protocols.

What this process is
A qualitative fit test is a documented check that a wearer can achieve an adequate seal with a specific respirator, and that they can keep that seal while moving and speaking. You test the individual, not the model in general; the record shows that this employee was assessed on this respirator, on this date, at this location. The checklist is built around three parts: the employee qualitative fit test record, a hazard assessment for construction dust and fiber particles, and the fit test exercises themselves. It is delivered in accordance with respiratory protection protocols, with 26 questions covering the test record, the hazard assessment, and the exercises.
Why it matters
A respirator only protects the wearer when it fits. In practice that means the face seal has to hold while the wearer moves, bends, and speaks, because that is what a shift demands of them. Two conditions can compromise the seal before testing even begins, and the checklist asks about both: - The wearer must be free from facial hair that could interfere with the respirator face seal. - The wearer must have donned the respirator independently without assistance. If either condition is not met, the fit test will not tell you what you need to know — that the wearer can achieve and keep a protected seal on their own. Before the exercises start, you apply a sensitivity solution and establish a sensitivity test threshold level, so the outcome is judged against the wearer's own response rather than a general assumption.
What a good fit test covers
A complete qualitative fit test record captures the administrative facts, the hazard context, the wearer's condition, the exercises, and the outcome. The administrative facts include the date conducted, who prepared the record, the location, the time finished, and the employee's identification details, signature, and the inspection real start and end dates. The hazard context is a hazard assessment for construction dust and fiber particles, which anchors the test to the reason the respirator is worn. The wearer's side covers freedom from facial hair that could interfere with the face seal, and whether the wearer donned the respirator independently without assistance. The equipment is identified by the respirator's manufacturer/make and model number, so the record matches the respirator actually tested. The exercises are the substance of the test. You run them in sequence and rate each one pass or fail: 1. Normal breathing 2. Deep breathing 3. Turning head side to side 4. Moving head up and down 5. Bending over 6. Talking (reading aloud) 7. Normal breathing — final check A full record also captures the sensitivity solution applied and the sensitivity test threshold level, the total test duration in minutes, an overall fit quality rating, and any additional observations or notes. The final normal breathing check is not a formality — it confirms the seal still holds at the end of the sequence.
How to carry it out
Work through the record methodically so that nothing is left to memory. Start with the administrative fields: the date conducted, who prepared it, the location, and the employee's identification details. Record the respirator manufacturer/make and model number. Confirm the wearer's readiness. Check that they are free from facial hair that could interfere with the respirator face seal, and note whether they donned the respirator independently without assistance. Complete the hazard assessment for construction dust and fiber particles. Apply the sensitivity solution and record the sensitivity test threshold level. Run the exercises in order. For each one, observe and record a pass or fail: - Normal breathing - Deep breathing - Turning head side to side - Moving head up and down - Bending over - Talking (reading aloud) - Normal breathing — final check Close out the record: give the total test duration in minutes, set the overall fit quality rating, and write down any additional observations or notes. Sign the record and capture the time finished, together with the inspection real start and end dates. A signed, complete record is what lets you show that the test was carried out in accordance with respiratory protection protocols and that the outcome is traceable to the employee and the respirator tested.
