Peer Performance Evaluation

A peer employee evaluation is a structured way for colleagues to give feedback on a coworker's professional performance and workplace behavior. The people who work alongside an employee every day see things a manager may not: how reliably they show up, how they treat teammates under pressure, and whether they take ownership when something goes wrong. This checklist gathers those observations in a consistent format so they can support informed human resources decisions.

Work Performance & Professional Conduct

The central part of this evaluation assesses specific competencies rather than leaving everything to general impressions. You will rate your coworker against a defined set of behaviors, most of which use a Yes, No, or N/A scale, with a frequency scale for how often they go above and beyond their basic responsibilities.

Detailed Assessment & Recommendations

Beyond the scored questions, the form asks for written responses that provide depth and context for the ratings. These open questions are where a peer evaluation becomes genuinely useful for human resources.

Why peer feedback matters

Managers can only observe so much. Your perspective as a colleague fills gaps and helps ensure that decisions about the employee are based on a broader picture. When peer evaluation is conducted fairly and consistently, it benefits everyone involved.

Frequently Asked Questions

The checklist defines the scope of the assessment for you. It groups the questions into peer performance evaluation, work performance and professional conduct, and detailed assessment and recommendations. The sections for work performance and professional conduct and for detailed assessment encourage open commentary alongside the structured questions, so written observations carry the same weight as the scored answers.

Peer feedback forms support human resources decisions by capturing observations from people who work alongside the employee. This template asks how long you have worked directly with the coworker, how often they go above and beyond, and how their duties intersect with or support your department's operations. It also includes a scale of 1 to 10 for rating overall performance and contribution to the team, plus open questions for recommendations and constructive feedback.

The checklist uses a few consistent rating scales. Several items ask how often something occurs, with options of Rarely, Occasionally, Frequently, and Consistently. Others ask Yes, No, or N/A for statements about timeliness, work ethic, accuracy, accountability, communication, adaptability, professionalism, and collaboration. One question uses a 1 to 10 scale for overall performance and contribution, and several open questions ask for a written response.

Many sections of this checklist include an N/A option. If you have not directly observed the specific behavior or do not have enough information to judge it, choose N/A rather than guessing. The open comments sections are also the right place to explain why a particular question is not applicable to your working relationship.

The evaluation ends with reflection and ownership questions, including whether the employee acknowledges and admits mistakes and takes accountability for their errors and works to correct them. This gives the evaluator a chance to comment on the coworker's contribution to the work environment and their follow-through on correcting problems.
