PDCA Template
Utilize this template to systematically design an all-encompassing action plan while implementing the PDCA (Plan-Do-Check-Act) cycle. This PDCA template features a comprehensive data set relevant across various industries and can be adapted to different sectors. Fill in the necessary specific details for each component of the plan, addressing all elements chronologically according to the PDCA cycle, and include essential information such as project name, initiator, location, and start date of the plan.

How to Use This Checklist
- **Complete the PDCA Framework details.** Record who conducted the assessment, who prepared the plan, the location, the finished at time, and the real inspection start and end dates. These fields anchor the cycle to a specific time and place.
- **Work through the Plan stage.** Define the problem thoroughly and establish objectives, observe the problem's characteristics and relevance from multiple perspectives, analyze root causes and possible solutions, and outline a step-by-step action plan with goals, timelines, and an approach to follow.
- **Move through the Do and Check stages.** Execute the steps in the action plan while monitoring predicted versus actual outcomes, then verify results by gathering data and assessing them against the indicators you established.
- **Finish with the Act stage and Notes.** Standardize the adopted solution to prevent recurrence, document and evaluate the results to validate the change, add any additional notes to include, and sign the record.

Overview
The PDCA template helps you design and track a structured improvement cycle using the Plan-Do-Check-Act method. You use it to work through a problem or initiative systematically: define what needs to change, carry out the change, verify whether it worked, and lock in what succeeded so the problem does not return. The template provides a comprehensive data set that is relevant across various industries and can be adapted to different sectors. The template is organized into a PDCA Framework section followed by the four cycle stages plus a Notes section. Early fields capture the project context, including who conducted and prepared the assessment, the location, and the plan's start and end dates. You then complete the Plan, Do, Check, and Act sections chronologically, filling in the specific details each stage requires.
Why it matters
Improvement work often stalls because teams jump straight to a solution without understanding the problem, or because a change is never verified or standardized. The PDCA template forces a sequence: understand first, act second, and confirm before making the change permanent. Working through the cycle in order gives you a repeatable record you can compare against over time. Because nothing is claimed without evidence, the template keeps the work grounded in observations, data, and defined indicators rather than assumptions.
What a good PDCA record covers
A complete PDCA record tracks the same cycle from start to finish. The template's fields fall into three groups: - **Context**: who conducted the assessment, who prepared it, the location, and the real start and end dates. - **Plan**: identifying and defining the problem, observing its characteristics and details, analyzing its root cause, and outlining a step-by-step action plan with goals and timelines. - **Do, Check, and Act**: executing the plan, verifying results against indicators, and standardizing the solution to prevent recurrence. A record is only as strong as the detail you put into it. If the plan, the predicted versus actual outcomes, or the verification data are left vague, the cycle cannot be checked or repeated reliably.
Legal and standards context
The PDCA template does not name or require any specific regulation or standard. It is a general-purpose improvement framework that you adapt to your own organization's obligations. Where your workplace is subject to rules or certification schemes, treat the PDCA record as supporting evidence that you planned, carried out, and checked an improvement. The compliance requirements themselves come from the standards and regulations that apply to your site, not from this template, so confirm them against your own sources.
How to carry out the cycle
1. **Set the context.** Fill in the project name, initiator, location, and start date, along with the conducting and preparing details. This fixes the scope and timing of the cycle. 2. **Plan.** Use the four Plan prompts in order. Identify the problem and set objectives, observe its characteristics and assess relevance from multiple perspectives, analyze the root cause and possible solutions, then write out the action plan with goals, timelines, and an approach. 3. **Do.** Implement the action plan as written and monitor progress, comparing predicted outcomes with what actually happens. 4. **Check.** Gather data and assess results against the indicators you set during planning. 5. **Act.** Standardize the adopted solution so the problem does not recur, then document and evaluate the results to confirm the change was justified. If it was, repeat the whole cycle. 6. **Close out.** Capture any additional notes and obtain a signature.
Using it well
Because the template suits many sectors, adapt the wording of each prompt to your own operation rather than adding new categories. Keep the problem statement specific: a narrowly defined problem is easier to observe, analyze, and verify than a broad one. Record the indicators you will use to judge success during the Plan stage, before you execute, so the Check stage has something objective to measure against. When a cycle succeeds, standardize the solution so it becomes the new way of working, and use the Notes field to flag anything worth carrying into the next cycle.
