How to Use This Checklist

  • Peer evaluations gather structured feedback on a co-worker from the people who work most closely with them. This form asks a colleague to rate specific behaviors and work habits and to record observations that help human resources make informed personnel decisions.
  • The evaluation section covers concrete, observable aspects of a co-worker's performance. Confirm each statement and record the answer as Yes, No, or N/A using the scale provided on the form.
  • The reliability statements cover whether your co-worker arrives on time, works hard, meets delivery deadlines, and pays attention to detail.
  • Collaboration and conduct statements cover communicating well with others, collaborating well, adapting to changing priorities, behaving professionally, and treating you with respect.
  • Accountability statements cover whether you trust your co-worker's decisions, whether they are willing to admit mistakes, and whether they take responsibility for those mistakes.
  • The open questions ask you to describe how the employee's functions or services interact with your department, how you would describe their ability to work with you and others, how receptive they are to your demands, and how you would describe their personal communication and behavior when interacting with you or your department.
  • A final open question asks what recommendations you have to help strengthen the working relationship.

What this checklist is for

A peer evaluation form captures feedback from the people who work alongside an employee. Rather than relying on a single manager's view, it gathers a consistent set of observations from co-workers on how someone performs day to day — their reliability, quality of work, collaboration, communication, and accountability. This form structures that input so it can inform human resources decisions about a co-worker you work with closely. The `Formulario de evaluación` is designed for exactly this: a colleague records their assessment of a peer, and those responses are collected to support HR decisions.

Why it matters

Co-workers see working habits a manager may not: whether someone arrives on time, meets delivery deadlines, pays attention to detail, and treats others with respect. Peer feedback can surface patterns in day-to-day behavior, such as whether a colleague is receptive to demands from your department or whether they take responsibility for their mistakes. Collected in a consistent format, these views add context to HR decisions that performance data alone may not provide.

What a good evaluation covers

A well-designed form asks about specific, observable behaviors rather than vague impressions. This one organizes its questions around several areas: - **Reliability and work quality** — whether your co-worker arrives on time, works hard, meets delivery deadlines, and pays attention to detail. - **Trust and accountability** — whether you trust their decisions, whether they admit mistakes, and whether they take responsibility for them. - **Communication and conduct** — whether they communicate well, behave professionally, and treat you with respect. - **Adaptability and teamwork** — whether they adapt to changing priorities and collaborate well with others. - **Effectiveness** — whether they are effective in their work. - **Cross-department working relationship** — how the employee's functions or services interact with your department, how well they work with you and others, how receptive they are to your demands, and how you would describe their personal communication and behavior when interacting with you or your department. Beyond the rated questions, the form asks for recommendations on how to strengthen the working relationship. That open-ended section is where the most useful detail often appears: specific examples and constructive suggestions, not just a yes or no.

How to carry it out

Work through the form in order and give yourself enough time to answer thoughtfully. 1. Record when the evaluation was conducted, who prepared it, and the location. 2. Answer each of the evaluation questions — on timekeeping, work ethic, impact on your environment, delivery deadlines, attention to detail, trust in decisions, admitting and taking responsibility for mistakes, communication, adaptability, professional behavior, collaboration, effectiveness, and respect. 3. Record your answer for the rated questions using the form's yes-no-not-applicable scale. 4. Answer the open questions about how the employee's functions interact with your department, their ability to work with you and others, their receptiveness to your demands, and their personal communication and behavior. 5. Write your recommendations for strengthening the working relationship. 6. Sign the form, record the time you finished, and note the actual start and end dates of the evaluation.

Answering fairly

Base your answers on what you have actually observed at work rather than on personality. When you use the yes-no-not-applicable scale, choose "N/A" only when a question genuinely does not apply to your working relationship; otherwise answer yes or no. In the open questions, describe specific situations and behaviors, and keep your recommendations constructive — the purpose is to help the reader understand the working relationship and act on it, not to settle a score.

Frequently Asked Questions

A peer evaluation form is a template used to collect structured feedback on a co-worker's performance from the people who work alongside them. It asks a colleague to rate specific behaviors and work habits, such as timekeeping, attention to detail, communication, collaboration, and accountability, and to record open-ended observations and recommendations.

Feedback from peers captures day-to-day working behavior that a manager may not observe directly, such as reliability, teamwork, and how someone responds to changing priorities. Gathering it in a consistent way gives human resources a broader evidence base when making personnel decisions, rather than relying on a single manager's view.

The main sections cover logistics (date conducted, who prepared it, and location) and the evaluation itself. The evaluation combines closed questions answered Yes, No, or N/A with open questions about how the person's work interacts with another department, how receptive they are to requests, and what recommendations the reviewer has for strengthening the working relationship.

At the start, record when the evaluation was conducted, who prepared it, and the location. When you finish, add your signature, the time you completed it, and the actual start and end dates of the evaluation so the record is fully traceable.

Focus on observable behavior rather than personality. Answer each question with Yes, No, or N/A, and use the open questions to give concrete examples and constructive recommendations. Being specific and fair helps human resources interpret the feedback correctly.
