How to Use This Checklist
- Record the assessment date, who prepared it, and the location.
- Classify the item category as a product, ingredient, or raw material, then name the item and describe its intended use and application.
- Screen likelihood by answering whether the material has been previously associated with documented food fraud incidents and whether there have been recent fraud alerts, recalls, or regulatory notifications related to it.
- Assess whether the material has inherent characteristics that increase its vulnerability, such as high value, a complex supply chain, or ease of adulteration.
- Record the overall pre-screening likelihood of fraud as the assessor's rating.
- Screen consequences by considering whether fraudulent adulteration or substitution could create public health or safety hazards, significant financial losses or commercial damage, or additional consequences such as brand reputation damage, regulatory non-compliance, or legal liability.
- Record the overall pre-screening impact severity as the assessor's rating.
- Add any additional assessor comments, observations, and supporting rationale, and sign off to complete the record with the finished time and the inspection's real start and end dates.

What Is This Food Fraud Vulnerability Assessment Template?
This template guides Quality Assurance Officers through a structured initial screening to evaluate the vulnerability of products, ingredients, or raw materials to food fraud. It supports a proactive assessment of both the probability of fraudulent activity and the potential severity of consequences should fraud occur.
Why Food Fraud Screening Matters
Food fraud can affect an item in more than one way. Fraudulent adulteration or substitution may create public health or safety hazards. It may also cause significant financial losses or commercial damage to the organization. Beyond that, it can lead to brand reputation damage, regulatory non-compliance, and legal liability. Screening materials for these possibilities before an incident occurs lets you decide where attention and mitigation are needed.
What the Initial Screening Covers
The screening begins with basic context — when it was conducted, who prepared it, and the location. You then classify the item category as a product, ingredient, or raw material, name the item being assessed, and describe its intended use and application. From there, the template examines two dimensions: - **Likelihood of Food Fraud Occurrence** — whether the material has been previously associated with documented food fraud incidents; whether there have been recent food fraud alerts, recalls, or regulatory notifications related to it; and whether the material possesses inherent characteristics such as high value, a complex supply chain, or ease of adulteration that increase its vulnerability. This concludes with an overall pre-screening likelihood of fraud, as rated by the assessor. - **Potential Consequences of Food Fraud** — whether fraudulent adulteration or substitution could create public health or safety hazards; whether fraud involving the item could result in significant financial losses or commercial damage to the organization; and whether there are additional significant consequences such as brand reputation damage, regulatory non-compliance, or legal liability. This concludes with an overall pre-screening impact severity rating. The template closes with a section for additional assessor comments, observations, and supporting rationale, followed by a signature and completion details.
Answer Scales Used in the Assessment
The template uses defined answer scales so that responses stay consistent: - **Incidents and alerts** are answered as *Yes - many incidences*, *Yes - few incidences*, or *No*. - **Inherent characteristics, and health or safety hazards**, are answered as *Yes*, *No*, or *N/A*. - **Overall pre-screening likelihood and overall pre-screening impact severity** are each rated as *Relatively Likely* or *Relatively Unlikely* by the assessor.
How to Carry Out the Screening
Work through the template in order, answering each question based on the evidence you hold for the item under assessment. Complete the context fields first, then the likelihood questions, then the consequence questions. When you reach the overall ratings, base them on the full picture rather than any single answer. Use the additional comments field to record observations and supporting rationale so the rating can be understood later. Then sign and close the record with the finished time and the inspection's real start and end dates. The result is a pre-screening judgement you can use to decide which materials warrant closer attention, further assessment, or mitigation.
