FMEA Template
A Failure Mode and Effects Analysis (FMEA) template is designed to identify potential issues within a process, product, or design, aiming to prevent adverse impacts on customers. This template allows you to track the implementation and effectiveness of corrective actions, enabling your team to clearly describe process functions, identify failure mechanisms, and assess risk prioritization numbers. Create and complete corrective measures, and validate the FMEA efficiently.

How to Use This Checklist
- Record the inspection details: the date it was conducted, who prepared it, the location, and the start and end times.
- For each item under review, write a function description and list the potential failure modes.
- For each failure mode, record the potential effects of failure, the potential cause of failure, and the current controls.
- Assign a severity rating, an occurrence rating, and a detection rating.
- Use the ratings to work out the risk prioritization number and decide which failure modes to address first.
- Create corrective measures for the highest-priority failure modes.
- Track the implementation and effectiveness of those corrective actions, and validate the FMEA.
- Sign the completed record and note the finish time.

Inspection
The inspection section captures the practical details of the review so the analysis can be traced to a time, place, and person. It includes: - Conducted on - Prepared by - Location - Signature - Finished At - Inspection real start date - Inspection real end date Fill these in as you begin, and close them out when the review is complete.
Failure Mode and Effects Analysis (FMEA)
Work through each item under review and record the analysis in full: 1. Potential Failure Mode — how the item could fail. 2. Potential Effects of Failure — what would happen if it did. 3. Severity Rating — how serious those effects are. 4. Potential Cause of Failure — why the failure might occur. 5. Occurrence Rating — how likely that cause is. 6. Current Controls — what is already in place to prevent or detect the failure. 7. Detection Rating — how likely the controls are to catch it. These ratings feed into the risk prioritization number. Use it to rank the failure modes and decide where corrective action is needed first.
FUNCTION
Start with the function description. Before you can analyse how something might fail, you need a clear statement of what it is supposed to do. The function description sets the standard against which each potential failure mode is identified. A precise function description makes the rest of the analysis sharper, because every failure mode should represent a specific way that function could be unmet.
Tracking and validating corrective actions
Once the risk prioritization number has highlighted the failure modes that need attention, create corrective measures and record how they will be implemented. Use the template to track implementation and effectiveness, so the team can see whether the action actually reduced the risk. Review the FMEA as the corrective measures take effect and validate that the analysis still reflects the current process, product, or design.
