What an employee performance appraisal is
An employee performance appraisal is a structured way to assess how someone has performed over a defined review period. You begin by recording the employee's details — their personnel number, hire date and employment status — and by setting the review period with a start and end date. You also identify who is conducting the appraisal. This administrative groundwork matters because it fixes exactly what is being assessed and over what span of time, so the appraisal reflects observed performance rather than a general impression.
Why a consistent set of criteria matters
A good appraisal scores the same criteria for every employee, using the same scale. Here, each criterion is rated from 1 (lowest) to 5 (highest), and you are asked to rate the employee objectively and on the basis of performance observed during the review period. That consistency is what makes results comparable, and it helps you do two things at once: recognise what is praiseworthy and identify where improvement is needed. Because the appraisal is also linked to a scoring sheet, the individual ratings add up to a total score that drives an overall performance rating — a single summary of performance for the period.
What the appraisal covers
The criteria span the core dimensions of day-to-day work: - [Attendance](#/checklists/generic/1) - Professional knowledge and skills - Quality of work - Productivity - Initiative and commitment - Teamwork - Team goals - General conduct - Discipline Alongside the ratings, the form captures any special tasks or awards the employee received during the review period and general comments on performance. It then records the agreed development measures, with the improvement measure, the person responsible and a target date. Signatures and completion details close the process.
How to carry out the appraisal
1. Record the employee's details: personnel number, hire date and employment status. 2. Set the review period (from and to) and enter the name of the appraiser. 3. Read the rating criteria carefully. 4. For each criterion, choose the applicable score on a scale from 1 (lowest) to 5 (highest), rating the employee objectively and on observed performance during the review period. 5. Add up the points to obtain the total score, and complete the overall performance rating. 6. Note any special tasks or awards and add your general comments on performance. 7. Agree the development measures, assigning the person responsible and a target date. 8. Record the signature and the completion details, including the actual start and end of the appraisal.
Making it work for your organisation
The template is fully customisable: you can adapt the criteria and the standards to match the ratings your company normally uses. If your organisation already applies a particular scale or set of criteria, align the form to it so that appraisals stay consistent across teams and comparable over time.
