How to Use This Checklist

  • Complete the details at the top of the form: the date it was conducted on, who prepared it, the location, the evaluator's name (optional), and the number of team members being evaluated.
  • Work through the team performance and team collaboration and dynamics criteria, rating each on the 1 to 5 scale based on your direct observations of the team.
  • For each team member, rate the individual performance criteria covering attendance, punctuality, active listening, commitment to the project, problem-solving, handling of disagreements, communication of decisions, reliability, and initiative.
  • Give an overall rating for each individual's contribution to the team's success and for the team's overall performance and effectiveness.
  • Answer the descriptive question describing the most valuable contribution each team member made to the project.
  • Sign the form and record the real start and end dates and the time you finished, so the evaluation is clearly identifiable and the schedule records match the actual evaluation period.

What this form is for

This form gives you a consistent way to assess how a team and its individual members performed over a project or working period. It covers the whole team's performance, each member's contribution, and the quality of collaboration and dynamics between them. It is not a scoring exercise for its own sake: the purpose is to produce a fair, documented picture of what the team achieved and how it worked together, so that strengths and problem areas can be identified and discussed. It can be used during the project lifecycle to check progress, or at key points such as a project close, a retro, or a performance review cycle. An evaluation is worth doing because team output depends on more than individual skill; it also depends on how decisions are made, how conflict is handled, and whether effort is shared fairly. Without a structured record, these things are easy to miss or dispute later.

What a good evaluation covers

A useful evaluation looks at the team as a whole and at each member individually, and it separates observable behaviour from overall judgement. The criteria in this form cover: - Team performance. The quality and inclusiveness of team decision-making, cooperation and mutual support, conflict management, fairness and balance of participation, focus and schedule discipline, internal communication, support provided to teammates, and overall team spirit and cohesion, each rated on a 1 to 5 scale. - Individual performance. Attendance at scheduled meetings, punctuality, active listening and receptiveness to others' perspectives, prioritisation of the project and acceptance of responsibilities, problem identification and resolution, engagement in disagreements and adaptability, keeping the team aligned on decisions and solutions, task reliability and meeting agreed deadlines, showing initiative without prompting, and an overall rating of each member's contribution. - Team collaboration and dynamics. The same practical questions of how members cooperate, communicate, handle conflict, and share the work, so you can compare how individuals contribute against how the group functions as a whole. The form also includes an overall team performance and effectiveness rating, a descriptive question asking you to describe the most valuable contribution each team member made to the project, and a short set of administrative fields (conducted on date, prepared by, location, evaluator's name, number of team members being evaluated, signature, finished at time, and real start and end dates).

Assessment scales

The rated criteria in this form use a 1 to 5 scale. The form defines the meaning of the scale so that evaluations are comparable; use the definitions the form provides and apply them consistently across the team. Rate what you observed rather than what you believe, and use the same standard for every member you assess so that the results are fair.

How to complete the form

1. Enter the administrative details first: the date the evaluation was conducted on, who prepared it, the location, the evaluator's name (optional), and the number of team members being evaluated. 2. Complete the team performance evaluation. Rate each team-level criterion on the 1 to 5 scale based on your direct experience of the team during the period being assessed. 3. Complete the individual member performance assessment for each team member. Use the same criteria and the same scale for everyone so that the ratings are comparable. 4. Give an overall rating for each individual's contribution to the team's success, and an overall rating for the team's performance and effectiveness. 5. Answer the descriptive question describing the most valuable contribution each team member made to the project. Be specific and refer to actual contributions rather than general impressions. 6. Sign the form, record the time you finished, and enter the real start and end dates of the period being evaluated so the record matches the actual evaluation period. 7. Where the form is used to support a performance conversation, share the completed evaluation appropriately and use it as the basis for a two-way discussion rather than as a verdict.

Using the results

The value of the form comes from what you do with it afterwards. The overall ratings show where the team stands; the individual ratings show where specific support, recognition, or development might be needed; and the comments explain the numbers. When you review the results, look for patterns across the team, such as recurring problems with deadlines, communication, or sharing the workload, and for mismatches between how the team is perceived and how individuals are perceived. Set out findings in plain language and always provide a written evaluation so the assessment is transparent and can be referred to later.

Frequently Asked Questions

Answer on a 1 to 5 scale, with 1 being poor and 5 being excellent, for the rated criteria. For the descriptive question, describe the most valuable contribution each team member made to the project.
