Fitness Testing Form

Comprehensive fitness assessment tool designed to evaluate body composition, muscular strength and endurance, joint flexibility, cardiovascular capacity, and postural alignment. This form enables trainers, coaches, and health professionals to establish baseline metrics, identify muscular imbalances, personalize training programs, and monitor client progress across repeated assessments.

How to Use This Checklist

  • Measure skinfold sites using the 7-site Jackson-Pollock method: triceps, pectoral/chest, midaxillary, subscapular, abdominal, suprailiac, and quadriceps/thigh. Record each in millimetres.
  • Take girth circumference measurements: on the left side with muscles flexed (neck, chest, waist, hip, upper thigh, mid thigh, calf, arm/biceps, forearm, shoulder) and on the right side relaxed (same sites). Record all in centimetres.
  • Perform muscular endurance and strength tests: sit and reach (cm), push-up repetitions, sit-up/crunch repetitions, and vertical jump (standing reach height, gross jump height, net jump height in cm).
  • Assess range of motion in degrees for flexion, extension, adduction, and abduction at the hip, knee, ankle/foot, shoulder, spine, and trunk. Use the provided fields for each joint action.
  • Record resting vital signs and conduct cardiovascular assessments using a cycle ergometer test and an aerobic fitness test. Document results in the appropriate sections.
  • If additional tests are needed, use the custom test fields to name the test and record the score or result.

Why Fitness Testing Matters

Fitness testing gives you a structured way to capture where a client is starting from. Without baseline data, programming becomes guesswork. This form collects objective measurements across multiple domains—body composition, strength and endurance, flexibility, cardiovascular capacity, and postural alignment—so you can see the full picture rather than relying on a single metric. A comprehensive assessment helps you identify muscular imbalances, such as a strength difference between left and right sides or limited range of motion in one joint compared to its opposite. These findings directly inform exercise selection and load progression. Repeating the same measurements later turns the form into a progress-tracking tool, showing whether your programming is producing the intended adaptations. This form is designed for trainers, coaches, and health professionals who need a standardised, repeatable protocol. By following the same procedures each time, you reduce measurement error and make comparisons across sessions meaningful.

Frequently Asked Questions

The form covers a 7-site Jackson-Pollock skinfold assessment, girth circumference measurements taken on the left in a flexed position and on the right in a relaxed position, muscular endurance and strength tests including push-ups, sit-ups, sit and reach, and vertical jump, range of motion at the hip, knee, ankle, shoulder, and spine in flexion, extension, adduction, and abduction, and resting vitals with cardiovascular tests using a cycle ergometer and an aerobic fitness test.

The form uses rating scales such as Optimal Alignment, Mild Deviation, Moderate Deviation, and Significant Deviation to categorise observations, alongside a range of numerical measurements and performance scores. This combination allows you to record both objective data and qualitative assessments of postural alignment and movement quality in one document.

Take each skinfold measurement at the seven sites using the Jackson-Pollock method, record girth circumferences on both the flexed left side and the relaxed right side, and capture performance scores for tests like the sit and reach, push-ups, sit-ups, and vertical jump. Range of motion is recorded in degrees for each joint action, and resting vitals are noted before any cardiovascular test.

Yes. The form includes a custom test field where you can enter the name or description of any additional test and record the corresponding score or result, making it adaptable to different settings and client groups.

Complete the fitness testing form when you need baseline metrics for a new client, when you want to identify muscular imbalances or postural deviations, when you are designing or adjusting a training program, or when you are tracking progress across repeated assessments over a period of time.
